The NHS Wales Shared Services Partnership, hosted by Velindre NHS Trust are acting on behalf of NHS Wales Executive's Strategic Programme for Urgent and Emergency Care who are seeking to establish a service to support the management of patients by the Clinical Support Hub (CSH) and located within the regional areas covered by the CSH.

II.2.4) Description of the procurement
The Strategic Programme for Urgent and Emergency Care are seeking to establish a 12 month pilot for a support contract which would carry out observations and assessments. This would enable the Clinical Support Hub (CSH) to manage more cases more appropriately saving time and resource for traditional UPC OOH services. The pilot provider are able to visit patients in their own homes and in care homes and there is also an option to have a wheelchair-enabled car that can transport patients either to hospital, if required, or to an out of hours base. An example of the activities under the Clinical Support Hub Mobile Resource Pilot are;
1. 12 lead ECG for interpretation by a Clinician
2. Taking bloods and transporting to laboratory
3. Urine dip
4. Weighing patients
5. Patient transport to UPC base
6. Support to pull cases from the 111 PEQ (stack)
7. Support direct admission
8. Patient follow up to monitor /update observations
The proposed pilot will cover the three CSH: Vantage Point House (East), Thanet house (West), and Ty Elwy (North), with two cars for North and West Wales to take account of the geography, thereby covering the whole of Wales. The service would be in place for 10 hours per day on Saturdays/Sundays and 8 hours on Bank holidays.

Justification for selected award procedure:
The procurement falls outside the scope of application of the Directive
Explanation
The support offered by St John’s has already been tried and tested by WAST and Health Boards and the offer can be flexed to include the management of falls in line with a Falls Response Framework and associated pathways if appropriate.
St John's has capacity to provide 2 vehicles along with the equipment and personnel who have the medical training, qualifications and capability to run a 12 month pilot scheme which is anticipated to commence mid November 2024 in time for supporting winter pressures. The pilot will be evaluated to understand if there have been any clear outcomes to providing this additional support and if successful, funding may be allocated for future requirement and therefore, there will be a new procurement process for a longer term arrangement.
