The Health Board is BCUHB is seeking a provider to deliver to provide practical support to older people who are ready for discharge home from hospital and whom require assistance and support to return to a safe, accessible home environment that enables them to live as independently as possible and reduce the risk of unnecessary re-admissions.

This service provider is expected to provide caseworkers based on the 3 hospital sites to work closely with hospital staff, patients and their families / carers to identify eligible, clinically optimised, patients at the earliest opportunity and put in place the necessary support that ensures a safe home environment for them to return to, facilitating timely discharge, avoiding unnecessary delays.
