To provide local access to equitable, prompt, consistent and high quality complex nursing assessment beds during 2014/15.
The Provider will accept direct referrals from the acute Provider, and work in partnership with them to facilitate the transfer in to the complex assessment beds and then to co-ordinate the patients' discharge into their future place of residence.
