The Provider will deliver a Shared Lives Scheme in Dudley that:- Will be provided for all adult person’s assessed as being in need of care and support under the Care Act 2014 Eligibility Criteria, who live in the borough of Dudley, this includes adults 18 upwards with complex needs, mental ill health and older adults.- Will provide support and accommodation to adults providing them with the opportunity to live in a family home with appropriate care and support.- Provide support for young people preparing for adulthood - The Scheme will attend Education and healthcare Plan (EHCP) reviews, Looked After Children (LAC) reviews and Pathway Plan Reviews to provide information and advice on Shared Lives for young people from 14+ as part of the initial planning. The Scheme will offer Short-Breaks post 16+, enabling the young adult to experience and become familiar with a Shared Lives Arrangement before they reach the age of 18.
