Abstract

To Improve outcomes for the individual and help prevent future crisis, whilst helping to reduce demand for health care, social care, housing and other statutory services.

The service will seek to understand the underlying causes of people's problems with the aim to help the individual to better deal with the symptoms to their challenges offering a holistic advice and support service thus equipping individuals wherever possible to make alternative choices to deal with their issues themselves and empower them to self-manage in the future.

The provider will seek to understand where, when and how demand presents, to ensure that the service continues to meet the needs of individuals with the outcome of improving people’s lives both mentally, economically, physically or environmentally.
