II.2.4) Description of the procurement
To Improve outcomes for the individual and help prevent future crisis, whilst helping to reduce demand for health care, social care, housing and other statutory services.
The service will seek to understand the underlying causes of people's problems with the aim to help the individual to better deal with the symptoms to their challenges offering a holistic advice and support service thus equipping individuals wherever possible to make alternative choices to deal with their issues themselves and empower them to self-manage in the future.
The provider will seek to understand where, when and how demand presents, to ensure that the service continues to meet the needs of individuals with the outcome of improving people’s lives both mentally, economically, physically or environmentally.

This procurement is being managed by Capita Business Services Ltd (‘Capita’) on behalf of the 5 Councils under a Strategic Partnership. Capita and the 5 Councils are delivering this procurement on behalf of the respective member of the 5 Councils Partnership.
The contract between the respective Council (Lead Purchaser) and Capita provides for tenders and all information submitted in response to this invitation to tender to be treated as confidential by Capita and not to be used for any other purpose than the provision of services to the Council under the Strategic Partnership.
The partnership has an established process for managing potential conflicts of interest should a partner wish to bid, and will invoke its ethical walls protocol when it is required. Details of this protocol are available on request.
