NHS Wakefield Clinical Commissioning Group (WCCG) is seeking to establish a contract with one experienced and established consultant for its Care Closer to Home requirement.
Wakefield CCG wants to look at the difference in outcomes experienced by Wakefield residents who experience care through the integrated care teams and compare this cohort with those who haven't experienced integrated care.
The health and social care community in Wakefield wish to put people at the heart of their services and therefore want to carry out meaningful and ongoing patient and public engagement.
Therefore, Wakefield CCG wishes to commission an independent, high-quality, evaluation.

Short Description
Health and social care in Wakefield is being transformed in order to provide care closer to people's homes, reduce unnecessary hospital admissions and support the timely discharge of patients. A full scale service redesign is taking place to create integrated, multi-disciplinary care teams in the community which will reduce unnecessary hospital admission and facilitate early discharge.
A number of positive outcomes have been identified, which will be achieved by this transformation, including seamless care for patients, improved independence for patients, increased, more timely support for carers, better staff experience and a more cost-effective health and social care system. Wakefield have developed an Assurance Framework covering the outcomes and measuring them using a number of relevant indicators. This is presented to the Programme Board on a quarterly basis.

Care Closer to Home Patient Engagement |

Short Description
Health and social care in Wakefield is being transformed in order to provide care closer to people's homes, reduce unnecessary hospital admissions and support the timely discharge of patients. A number of positive outcomes have been identified, which will be achieved by this transformation, including seamless care for patients, improved independence for patients, increased, more timely support for carers, better staff experience and a more cost-effective health and social care system.
The health and social care community in Wakefield wish to put people at the heart of their services and therefore want to carry out meaningful and ongoing patient and public engagement.
Conventionally, measuring people's experience of services and service change tends to be ad hoc, conducted by staff and rely heavily on patients and the public taking the initiative to attend a focus group, event or complete a written survey.
There are a number of weaknesses with this approach:
— Surveys are not ideal for a population such as Wakefield, with low literacy levels and often lack validity due to poor response rates. Surveys are also seldom representative of all groups, as those people who are more highly educated, and therefore less likely to suffer from poor health, are more likely to complete them. They are also more likely to be completed by people that have had a very bad or a very good experience of services, making them vulnerable to bias.
— Qualitative interviewing is valued for the rich data it accesses about people's experiences of services. It can focus on lived experiences and allow more profound conclusions to be drawn by the reflective process. It is also ideal for those who have low literacy levels. This can be really useful for decision makers when shaping services and understanding people's behaviour. However, it is unrepresentative of a wider population and uses a very small sample, unlike written questionnaires.
— Ad hoc and one-off consultations are limited when evaluating on-going services particularly if you are trying to continuously develop them in response to patient and public involvement.
— Consultation carried out by staff run the risk of being biased as patients, particularly older ones, worry that they might be branded troublemakers if they give negative feedback (a recent report (April 2013) by the Care Quality Commission 'Fear of Raising a Concern about Care', showed that 26 % of people were worried that complaining would brand them as a 'trouble maker').
