The contract is to appoint a Lead provider to deliver the Families Team element of the Resilient Families Service (RFS).
The team will work with families assessed as requiring early interventions where coordinated and multi-agency support is required. The Families Team will also receive cases transferred from the other RFS teams on completion of successful outcomes to provide support to build resilience. The team will have a good understanding of evidence relating to promoting change in families and consider the known risk factors that may occur during this process.
The Families Team will comprise of Intervention Workers, who will provide a central point of contact and advocate for families and provide direct support.
The Families Team Intervention workers will work alongside partners to ensure the following;
1. The intervention plan is effectively delivered including the co-ordination of professionals and services involved in the delivery of aspects of the plan
2. To be the central contact point for the family and all professionals and services involved in the plan and address any issues that risk disengagement from the process
3. The team are family focused and employ a strength based approach that empowers families to identify their own solutions and recognise the distance travelled
4. The team actively encourage the ongoing engagement with universal services
5. Where a family is open to the team and a Police Protection Notice is received, the initial child protection check is undertaken by the intervention worker.
